Blepharoplasty with laser eyelid resurfacing combines the precision of eyelid surgery with advanced CO₂ or Er:YAG laser technology for comprehensive eye rejuvenation. Patients with ocular rosacea have dilated blood vessels known as telangiectasia along the eyelid margin, which contributes to increased eyelid inflammation and can cause meibomian gland dysfunction.  BBL causes photocoagulation of these abnormal blood vessels, helps open the meibomian gland, and liquifies the oil within the gland.

WHAT TO EXPECT:
Treatment requires protective contact lens placement.  There may be mild swelling, redness or crusting of the area for 5-7 days after treatment.   Most patients notice an improvement within days of having the first treatment.  3-4 treatments are recommended at 2-3 week intervals with subsequent yearly touch ups.

WHAT TO EXPECT?

By making an incision along the natural eyelid skin fold, it is possible to remove skin and fat and leave only a fine, discrete scar along the natural skin crease of the eye. Excess upper eyelid skin can lead to functional problems in the eye; causing a loss of peripheral vision, especially in the outer and upper parts of the visual field. Initial bruising and swelling resolve in as little as one to two weeks, however, the final outcome of the surgery may not be seen for several weeks.

On the day of surgery, the amount of skin to be removed is carefully marked in an upright position and then rechecked with the patient reclined to ensure that the eyes will close without difficulty after surgery. A local anaesthetic is injected under the skin and the incisions are placed within the eyelid. The procedure generally takes 30-60 minutes. Fat pads that are normally present in the eyelids can be trimmed or repositioned to contour the eyelid for a youthful appearance. Non-dissolvable sutures are placed under the skin to minimize scarring, and bandages may be placed to secure the sutures. These sutures will be removed 1 week later.


Immediately after surgery, you may notice your vision is blurry due to the antibiotic ointment that has been applied during surgery. You will be asked to use this ointment at bedtime in the eye and 
on the wound. Artificial tears will be prescribed to prevent dryness in the postoperative period.
The degree of swelling and potential bruising differs from patient to patient. We will give you cold compresses to use for 10 minutes every hour while awake for the first 48 hours to minimize bruising and swelling. It is encouraged to sleep with 2-3 pillows to help prevent fluid accumulation
and swelling.

Regular daily activities can resume the day of or after surgery, but exercise and strenuous activity should be avoided until after the first postoperative visit. I recommend no driving until after the sutures have been removed. Patients return to 1 week after surgery for examination and suture removal. Eye make up can be resumed 7-10 days after surgery. It is normal to have feelings of numbness along the eyelash margin, tightness of the eyelids and mild dryness temporarily. Most patients do not experience any pain after the surgery.

It is important to keep the eyes dry for the first 48 hours. Hair can be washed. The eyes can be gently washed from days 3-7 and once the sutures are removed at one week, normal washing routines are initiated.

Serious complications after upper eyelid blepharoplasty are rare. However, there are potential
risks and complications as with any surgery: infection, bleeding, and poor wound healing. In an experienced and conservative surgeon’s hands, complications such as the inability to close the eyes or vision impairment are extremely unlikely. A detailed discussion of all possible risks and complications is made at the time of operative consultations and detailed with the written information summarizing discussions.

Blepharoplasty is designed to improve the appearance of the eyelids and most patients do not feel the need to have a second surgery. However, the natural ageing process will continue and results will vary between patients. Blepharoplasty results last between 7-15 years. Blepharoplasty can be enhanced by other surgical and non-surgical procedures done at the same time.

Can you do a blepharoplasty with laser?

Yes. You can combine eyelid surgery with laser resurfacing around the eyes in a single procedure. It’s typically performed by highly experienced oculoplastic or facial plastic surgeons like Dr Maryam Zamani, due to the delicate eyelid anatomy and the power of the lasers used.

Is blepharoplasty with CO2 laser better than standard blepharoplasty?

Laser resurfacing can improve results for some patients. However, the best outcome usually depends more on appropriate patient selection and surgeon expertise. CO2 laser can target skin laxity, particularly on the lower eyelid, creating a softer, smoother lower eyelid without the scar of an incision point when skin is removed from the lower eyelid (does that make sense?).

Which laser types are used for eyelid resurfacing (CO₂ vs Er:YAG)?

For eye resurfacing, the most commonly used lasers are ablative CO₂ and Er:YAG, sometimes combined with a non-ablative wavelength such as 1540 nm. Both CO₂ and Er:YAG can deliver beautiful, natural results when used appropriately. Ultimately, outcomes depend less on the laser itself and more on the surgeon’s technique and experience. Er:YAG treatments are typically more superficial, offering gentler tightening and a faster healing time.

What concerns does blepharoplasty with laser eyelid resurfacing treat?

Laser eyelid rejuvenation paired with surgery can target:

What are the risks or complications of combining laser resurfacing with blepharoplasty?

Risks can include prolonged redness, swelling and bruising, temporary irritation or dry eye symptoms, infection, changes in skin pigmentation, scarring, and, rarely, eyelid malposition such as ectropion or difficulty fully closing the eyelid. Protective metal eye shields are typically placed in the eyes during laser eyelid treatment to help protect the ocular surface.

What are the disadvantages of laser resurfacing around the eyes?

Disadvantages can include more surface downtime than surgery alone, increased sun sensitivity during healing, pigment risks in deeper skin tones, and limited frequency for repeat strong ablative resurfacing. Results depend heavily on technique and aftercare.

How long does laser blepharoplasty last?

Results can last for many years. The effect includes immediate tightening and longer-term collagen remodelling over several months. Longevity varies with ageing, genetics, lifestyle, and sun exposure.

When will I see results?

Some improvement is visible once swelling settles, while collagen remodelling continues for several months after laser resurfacing. The final refinement is typically progressive rather than instant.

How long does the procedure take and what anaesthetic is used?

A combined laser eye lift approach with surgery and resurfacing often takes around 60 to 90 minutes, depending on complexity and whether upper lids, lower lids, or both are treated. It may be performed under local anaesthetic with sedation or under general anaesthesia, depending on the plan and patient needs.

How much does blepharoplasty with laser eyelid resurfacing cost in London?

Fees vary depending on whether upper, lower, or both eyelids are treated and by complexity. Combined blepharoplasty laser treatment with laser resurfacing is typically higher than blepharoplasty alone due to added laser time, consumables, and facility costs. When can I wear makeup again after laser eyelid resurfacing?
